This form is for Trail Patrollers to report their activities each month. Please contact the Volunteer Coordinator and the Trail Group Liaison immediately if there has been a serious incidence encountered or dangerous trail condition observed.

Trail Conditions: Please describe any problems with the condition of the trails you have patrolled. List the trail name, specific problem, and where the problem exists on the trail. e.g. washout, vandalism, trees down etc.
